Vish Anantraman, MD, Chief Information Architect for North Shore LIJ Health System, is reducing the costs of care by using a management system that increases the efficiency of care transitions (inpatient to outpatient to home care).
“We identify high risk patients earlier than traditional case management through an internal health information exchange. We aggregate information for over 75 systems including all of our EMRs, home care systems, and case management systems to review patient data in real time,” Anantraman says.
Once a diagnosis is made, the system alerts care coordinators who begin to plan discharge management at the time of admission. The system designs personalized post-discharge care for complicated, high-risk cases. “We are moving rehab from long-term care facilities and skilled nursing facilities to home care to provide a higher level of care at a lower cost,” he says.
Dr. Hochron co-founded and is the Chief Medical Officer of Practice Unite, a mobile solution that improves healthcare communications and provides healthcare systems with a platform for their mobile strategy. He received his MD degree from New York Medical College, and his JD degree from Rutgers Law School. He is a Clinical Professor in the Department of Medicine at UMDNJ-Rutgers medical school.
