Wellframe uses a mobile app to connect users to a care management dashboard. From there they are able to directly communicate with a clinical team that helps develop personalized care plans that can then be sent to a phone or tablet. The dashboard supplies care managers with real time information on their patients, which helps caregivers intervene earlier in the care process and thereby reduce the cost of care.
