The implementation of the Affordable Care Act (ACA) created a number of new opportunities for healthcare and community-based providers to partner together to address the three-part aim of better care, better health for populations and communities, and lower costs. An integral part of achieving that aim for older adults and persons with disabilities is a person-centered plan that addresses both their health care and long-term supports and services (LTSS) needs. Interoperable health information technology (health IT) systems that reach across settings and providers could not only enhance communication, and care coordination activities currently underway within ACA-funded programs, but also facilitate the development of an accessible, integrated, person-centered plan that can help individuals achieve their goals and enhance their quality of life.
This all-day workshop will discuss developing an integrated, person-centered plan enabled by health IT, including how to improve communication and collaboration among community-based organizations and health care partners.
